Restoring a near anatomical hip joint in an 18 year old neglected hip fracture dislocation – A rare case report

Long standing neglected traumatic hip fracture dislocation is a rare clinical condition. Obliteration of the native acetabulum, soft tissue contractures, loss femoral head congruity and associated fractures make this condition challenging to treat. We discuss case middle-aged male presenting us with pain, shortening inability bear weight on left lower limb for last 15 years. Clinical examination revealed an adduction deformity over hip, globular swelling in gluteal region fixed equinus ankle. Movements at hinted restricted internal rotation flexion. Pelvis radiographs posterior ipsilateral femur neck non union. Patient underwent initial upper tibial traction 8 weeks followed by hybrid Total Hip Arthroplasty. Postoperatively intensive physiotherapy was continued correction ankle patient able ambulate. On five years follow-up, mobilizing independently, pain-free mild short gait some residual foot drop. Treatment can be challenging. Associated economic restraints may hinder clinicians from higher imaging treatment even difficult. However, anatomical reconstruction total arthroplasty has yielded good functional outcomes long dislocations.’
